You may qualify if you receive Michigan Medicaid (including UnitedHealthcare Community Plan or Molina Healthcare) and need help with daily activities like bathing, dressing, or meal preparation.

Call or fill out our form and one of our enrollment specialists will guide you through the process — completely free of charge.

Complete a Needs Assessment
A Medicaid assessor will visit your home to determine the level of care you need. MiWay will help you prepare for this visit.

Choose Your Caregiver
Under the Home Help self-direction model, you choose who provides your care — including a trusted family member or friend.

Start Receiving Care
Once approved, your caregiver begins providing services and gets paid directly through the Medicaid program. No cost to you.
